Update 1: FAQs were added at the bottom of this page. PDF version of all these FAQs can be downloaded from this post.
Galaxy S4 dials music up and down, and balances left and right audio based on your hearing, the sound source and your preferences.
Samsung called this Adapt Sound.
Basically, audio can be adapted and optimized to the content you’re listening to (video, music, etc.) for your ears.
This is good for your ears.
To set a personalized sound while listening to songs with a headset, tap Settings—Adapt Sound-– On.
When you turn the volume up to level 14 or higher, the adapt sound option is not applied to music playback. If you turn the volume down to level 13 or lower, the option is reapplied.
This feature can be controlled in Settings–Sound–Adapt Sound.

For the ones who needs the adapt sound pattern for the stock S4 Headset(white flat-corded headset included in the S4 box), feel free to try the pattern I came up with..
001101001101100011000000110010111100
or
101011001011100001001011001010000010
Legend:
0: No
1: Yes
compare this to your adapt sound pattern and see if this one does better.
Most used side (left/right) is an option that is entirely based on your preference.
Enjoy..
Disclaimer:
Patterns may differ from octa/quad S4 versions.
I came up with this pattern on my
Galaxy S4(SHV-E330S) Quad Core “LTE-A” version.

Many of you are wondering how to disable the Adapt Sound feature and like me, have found little to no success in doing so after google searching and forum scrolling. I have a solution! Forum “experts” will try to tell you that you simply go to settings> device> sound> adapt sound to “reconfigure” adapt sound, but nobody will tell you the real answer… until now anyway. You need to go to Settings> More> Apps Manager>Running (the All section will work as well)>Adapt Sound. There you will be prompted with a few options, the ones you will look for being “Force Stop” and “Turn Off”. Both of these features will be able to be reversed if you think you will use Adapt Sound again, but if you are likely to toggle it on and off frequently, select the “Turn Off” option from the “Turned Off” section of the apps manager. This way your settings will be saved and you will not have to run the testing process each time you wish to toggle it on. However, if you attempt to start the process while the feature is turned off, the settings app will close. This should certainly help those of you who have been irritated by Adapt Sound.

Justin, I have a Tab 4 7.0. I haven’t been able to find ANY answers online about disabing Adapt Sound my tablet. Finall, in disgust, I went back to Adapt Sound and said “yes” to every question. It’s not optimal sound but at least I can hear vocalists in Amazon Music, etc., again. Do you have any ideas about setting up Adapt Sound on a Tab 4?

After disabling the app, you can try to reboot the phone.
Of course, you can always backup the phone and perform a factory data reset.
